Abstract

In Indonesia, the incidence of baby blues or postpartum blues is between 50-70% of postpartum women based on childbirth, the majority of respondents, almost half of them have cesarean deliveries, namely 11 respondents or 27.5% experiencing baby blues syndrome. One of the obstacles in breastfeeding at the beginning of a baby's life is due to the influence of childbirth. Some mothers do not give exclusive breastfeeding because their milk does not come out or only a little comes out so it does not meet the needs of their babies. Nationally, exclusive breastfeeding coverage in Indonesia is still low, 2010 Susenas data shows that only 33.6% of infants in Indonesia receive exclusive breastfeeding. This study aims to obtain an overview of the implementation of nursing care for clients who experience Postpartum Sectio caesarea with ineffective breastfeeding at dr. Esnawan Antariksa Hospital, East Jakarta. The research design used is descriptive qualitative with a case study approach. There are three types of participants in this study, namely: the client (two people), the client's husband (two people), and the room nurse. Data collection was carried out for three days from May 9 to May 11 2018. The data taken were interviews, observations, physical examinations and document review. Analysis using triangulation method. At the assessment stage, several parts were found that were not in accordance with the theory, namely respiration, body temperature, chief complaint, and breasts. At the nursing diagnosis stage, there is a gap between the theory and the case of Mrs. M and Mrs. R. In the case of Mrs. R found two nursing diagnoses, with one nursing diagnosis in accordance with the theory, namely comfort disorders: acute pain and high risk of infection, and two additional nursing diagnoses, namely ineffective breastfeeding. At the planning stage there is no gap between the literature review and in practice. The evaluation stage has been carried out in accordance with the theory, namely conducting formative and summative evaluations. Summative evaluation in the case of Mrs. M, the three issues have been resolved, and for Mrs. R, all three problems have been solved.
