Abstract

Pulmonary tuberculosis is an infectious disease that attacks the lung parenchyma, caused by Mycobacterium tuberculosis. The results of the study in 2016 the number of patients with pulmonary tuberculosis was 233 patients and in 2017 there was an increase in pulmonary tuberculosis patients by 267 people. This case study aims to obtain an overview of the implementation of nursing care for clients who experience ineffective airway clearance with pulmonary tuberculosis at RSAU dr. Esnawan Antariksa, East Jakarta. The study was carried out in the Merpati and Garuda rooms in March – April 2018 on both patients for 3 days. The research method used is a descriptive qualitative research method with a case study approach with an intervention that is teaching effective coughing. The participants in this case study were two clients, two family members of the client, and two room nurses. At the assessment stage, subjective data was obtained on both clients complaining of coughing up phlegm for more than 2 weeks. The objective data of both clients was cough, cough accompanied by yellow sputum. The main priority nursing diagnosis for both pulmonary tuberculosis clients is ineffective airway clearance related to the accumulation of secretions. At the planning stage of the priority diagnosis problem, a nursing action plan is determined that refers to the goals and outcome criteria. Based on the nursing diagnoses above, the nursing action taken was to teach effective coughing with a frequency of every 2 hours for 3 days. Evaluation of nursing problems in ineffective airway clearance related to the buildup of secretions is resolved marked by reduced coughing. The client appears to be able to produce sputum, vesicular breath sounds, respiratory frequency 20 times / minute so that the intervention is stopped. The conclusion is there is no gap between theory and facts obtained in nursing care for clients who experience ineffective airway clearance with pulmonary tuberculosis at RSAU dr. Esnawan Antariksa Jakarta.
